Community Feedback/Complaint Form

The City of Yucaipa maintains high standards of personal and professional conduct for its employees and is committed to providing quality programs and services to the community. Anyone wishing to file a complaint, community concern or has community feedback should complete and submit this Community Feedback/Complaint Form.

The information requested on this form is necessary to properly investigate the issue(s) raised. The submitted form will be examined on their own merit, and a member of the City will be assigned to contact the submitter within 15 days. In addition, the submitter will receive a written notice of disposition via email.

Please note that this form is not intended to file a complaint regarding overall City services, or report an issue pertaining to streets, parks, trails, code enforcement, homeless issues, graffiti/vandalism, streetlight or power outages or to submit a crime. To report these types of issues, please use the City of Yucaipa mobile app or the Report an Issue form.
